1. Complete a separate form for each Item ID.

2. Complete this for in its entirety. Be sure that all entries are complete and correct

3. Press Submit. The completed form is emailed to our RMA department.

4. A RMA# will be generated and a form will be either emailed or faxed to you.

5. Print or copy the form you recieve via email or fax and include in the package with your item.

6. Write the RMA# on the outside of the package.

7. Ship the item back via UPS, FedEx or other trackable service.
